Here are forms needed during the admissions process.  To apply: please complete a school application, release of student information and application fee.

We are currently accepting applications for grades 1-7 on a rolling basis for openings and for our wait pools.  Contact Susan for more information.

Application for admission

• Download the application PDF (110K) – return by mail

Admission application fee

• Pay the $60 admission application fee online
• Alternatively, you are welcome to mail a check for this fee 

Required Release of Student Information form

• Downloadable PDF (61K). A release of information form must accompany your school application. This form cannot be completed online — it must be downloaded, signed, and returned to the school by mail (1365 Englewood Avenue, Saint Paul, MN 55104), fax (651-917-0708) or in person.
